Derbyshire Mind Active Monitoring Practitioner Vacancy
Hours: 22 hours per week, worked on 3 days per week – days to be agreed. The post requires flexible working and may include occasional evening and weekend work.
Salary: NJC points 14-16. Starting salary point 14, £22,462 per annum pro rata.
Contract: 12 month contract.
Derbyshire Mind is one of a federated network of local Mind charities across the country. Our focus is on improving mental health and wellbeing for people across Derbyshire and Derby City. We provide a range of advocacy and community wellbeing services designed to support people with mental health problems as well as the wider population. We are committed to developing innovative and responsive services which further promote mental health awareness and wellbeing.
Active Monitoring is a psycho-educational programme designed to provide early intervention provision within Primary Care. The role will involve working directly from a GPs surgery, delivering the Active Monitoring programme. The role will proactively work with individuals on agreed pathways to support early intervention via a facilitated self-help approach.

We are looking for an enthusiastic and skilled individual to deliver the Active Monitoring programme from GPs surgeries in Derbyshire.
The successful candidate will have a Level 3 qualification in Health & Social Care or equivalent and experience of providing mental health/wellbeing services to clients within a primary care setting. They will have excellent interpersonal skills and be able to build strong working relationships with both internal and external stakeholders, as well as an enthusiastic, committed approach and personal resilience. The role will be based in 3 GPs surgeries in Derbyshire, and the postholder will be required to travel to each of these on a weekly basis.
